All policies issued by Health Republic Insurance of New York (“HRINY”) were terminated on November 30, 2015.

On May 11, 2016, the New York State Supreme Court entered a Liquidation Order placing the company into liquidation.

On October 17, 2016, the Court entered an Order, which approved the Claims Adjudication Procedure in this liquidation proceeding.

In accordance with the Claims Adjudication Procedure, the Explanation of Benefits/Allowance Voucher (“EOB”) serves as notice of determination of whether submitted claim is allowed or disallowed in the proceeding. The expanded description of the Reason Codes for EOBs is posted in the Key Documents section of this website. All EOBs were mailed in 2017 and the appeal procedures were outlined on each EOB. In accordance with the Claims Adjudication Procedure established for HRINY claims, the deadline for submission of an appeal of the Liquidator’s determination of a claim, and all agreed extended periods for the submission of an appeal, have expired, and no further appeals will be accepted. Timely filed appeals are currently under review and you will be notified of a decision upon the Liquidator’s determination.
